Diabetic Information Sheet We find this sheet invaluable.  It has been given to every daycare provider, school, grandma/grandpa, etc.  That way, we know that all the information needed is at their fingertips.

Gabe's Schedule: When he was an infant/toddler, we found it extremely important to make sure everyone kept him on the same schedule. This form helped.

Blood Glucose File (Page 1):  This form works wonderful when sending data to your endocrinologist via fax.  We frequently did not need to visit the endocrinologist, but wanted to run Gabe's numbers by the doctor to "tweak" his insulin regime. This has room for 7 days worth of "blood sugar" data.

Blood Glucose File (Page 2):  This is a continuation of page 1 if more than 7 days of data is needed.

Favorite Food List:  We formulated this list specific to the foods that Gabe ate frequently in order to count carbohydrates.  We found that this list hung on the refrigerator at grandmas house and at the daycare providers home as well!  (We still never left home without our carb. counting books.)'
Running Blood Sugar List:  For those that like a simple running list of blood sugars.

Section 504 Plan: We referred to this in the Diabetes Information section.  Gabe started kindergarten this year.  We spoke with the principal, and they were already familiar with this plan.  We filled this sheet out so that everyone knows how to take care of Gabe specifically.

Smart Chart for the Computer: If you use the "Pocket Pancreas" and "Smart Charts" to keep track of your insulin requirements, food intake, activity levels, ketone levels and glucose levels this computer form is for you.  I made this form up based on the Smart Charts so that I could fax my son's information to his endocrinologist.  It is a great communication tool.
